3. Development of Safety Infrastructures for the Petroleum Industry

(1) Safety Support Database System (PEC-SAFER)

The data available with PEC-SAFER are divided into five types:

1) Data on Hiyari-hatto Incidents (i.e. Potential Accident Experiences)/AccidentsData

Data have been collected regarding accidents and Hiyari-hatto incidents which were prevented before developing into accidents in refineries. The causes of these incidents/accidents are analy-zed and shown together with the associated pre-ventive measures. It is intended that the database is shared by the whole petroleum industry to help prevent other similar incidents/accidents. Cu-rrently, data on 320 Hiyari-hatto incidents and 401 accidents are accessible.

2) Data on safety training

In consideration of the decrease in the number of experienced engineers, safety training resources are provided for informing younger engineers of safety techniques as well as for supporting safety education in the petroleum industry. The database on safety training consists of 838 topics, includ-ing 173 topics on basic safety knowledge, 597 topics on operational knowledge/techniques, and 56 topics on web-based learning materials. Links are provided on the PEC-SAFER page to directly access web-based learning resources of other or-ganizations.

3) Data on facilities deterioration and execution management

The source of facility fuilaves causing accidents are divided into deterioration and the lock of quality in construction (e.g. weld flaws and flanges not properly seated.) The data on facilities deteriorations are not usually made public, and are avail-able on PEC-SAFER for sharing and disseminating information which is useful for preventing accidents caused by similar deteriorations. The data on execution management takes the form of guidelines for construction management through which owners of facilities can ensure reliable quality of execution. The database contains 153 deterioration cases and 23 topics of execution management.
